In order to receive points for your participation, please fill out the information below. All of the information MUST be filled out.

  • If you volunteered and donated blood, you will still only receive 4 points.
  • If you went to donate and did not qualify, you will still receive your 4 points as long as you showed up.
  • If you were a Super Volunteer, you will receive 1 additional point. (Super Volunteers went through training prior to the blood drive and had hats with superman buttons.)
